# Practice Financial Policy

## **Practice Financial Policy**

Thank you for choosing **Rudd Vision, PLLC** as your health care provider. We are committed to building a successful physician-patient  relationship, and the success of your medical treatment and care. Your understanding of our Practice Financial Policy and payment  for services are important parts of this relationship. For your convenience, this document discusses a few commonly asked financial  policy questions. If you need further information or assistance about any of these policies, please ask to speak with our Practice

Manager.

**When are payments due?**

All copayments, deductibles, patient responsibility amounts, and past-due balances are due at the time of check-in unless previous  arrangements have been made with our billing coordinator.

**How may I pay?**

We accept payment by e-check, ACH, VISA, MasterCard, Discover and American Express. We do not accept post-dated checks.

**Do I need a referral or pre-authorization?**

If your insurance plan requires a referral authorization from your primary care physician or a pre-authorization from your insurance,  you will need to contact your primary care physician or insurance company to be sure it has been obtained. If we have yet to receive authorization prior to your appointment time, we will reschedule. Failure to obtain the referral or preauthorization may result in a  lower or no payment from the insurance company, and the balance will be come the patient’s responsibility.

**Will you bill my insurance?**

Insurance is a contract between you and your insurance company. In most cases, we are not a party to this contract. We will bill  your primary insurance company on your behalf as a courtesy to you. To properly bill your insurance company, we require that you  disclose all insurance information, including primary and secondary insurance, as well as any change of insurance information.

It is your responsibility to notify our office promptly of any patient information changes (ie, address, name, insurance information) to facilitate appropriate billing for the services rendered to you. Failure to provide complete and accurate insurance inform ation  may result in the entire bill being categorized as a patient’s responsibility.

Although we may estimate what your insurance company may pay, it is the insurance company that makes the final determination of  your eligibility and benefits. If your insurance company is not contracted with us, you agree to pay any portion of the charg es not  covered by insurance, including but not limited to those charges above the usual and customary allowance. If we are out of ne twork

for your insurance company and your insurance pays you directly, you are responsible for payment and agree to forward the payment to us immediately.

**Which plans do you contract with?**

**Rudd Vision PLLC** accepts most major insurance plans. However, with the frequent changes that happen in the insurance  marketplace, it is a good idea for you to contact your insurance company prior to your appointment and verify if we are a  participating provider as per your plan.

**What if my plan does not contract with you?**

If we are not a provider under your insurance plan, you will be responsible for payment in full at the time of service. As a courtesy,  however, we will file your initial non-surgical insurance claim, and if not paid within 45 days, you will be responsible for the total  bill. After your insurance company has processed your claims, any amount remaining as a credit balance will be refunded to yo u.

**What is my financial responsibility for services?**

It is your responsibility to verify that the physicians and the practice where you are seeking treatment are listed as authorized  providers under your insurance plan. Your employer or insurance company should be able to provide a current provider listing.

**If you have:**

Workers’ Compensation

- _If we have verified the claim with your carrier_: No payment is necessary at the time of the visit.  • _If we are not able to verify your claim_: Your appointment will need to be rescheduled.

Our staff will schedule your appointment after your worker’s compensation carrier calls in advance to verify the accident date,  claim number, primary care physician, employer information, and referral procedures.

Workers’ Compensation (Out of State) and Occupational Injury

- Payment in full is requested at the time of the visit.

Our staff will provide a receipt to file the claim with your carrier.

The patient or the patient’s legal representative is ultimately responsible for all charges for services rendered. “Non-covered”  means that a service will not be paid for under your insurance plan. If non-covered services are provided, you will be expected to  pay for these services at the time they are provided or when you receive a statement or explanation of benefits (EOB) from your  insurance provider denying payment.

Your insurance company offers appeal procedures. We will not under any circumstances falsify or change a diagnosis or symptom to convince an insurer to pay for care that is not covered, nor do we delete or change the content in the record that may prevent  services from being considered covered. We cannot offer services without expectation of payment, and if you receive non -covered  services, you must agree to pay for these services if your insurance company does not. If you are unsure whether a service is covered  by your plan, ultimately, it is your responsibility to call your insurance company to determine what your schedule of benefits allows,  if a deductible applies, and your potential financial responsibility.

**What if I don’t have insurance?**

Self-pay accounts are used for patients without insurance coverage, patients covered by insurance plans which the office does not  accept, or patients without an insurance card on file with us. Liability cases will also be considered self-pay accounts. We do not  accept attorney letters or contingency payments. It is always the patient’s responsibility to know if our office is participating in  their plan. If there is a discrepancy with our information, the patient will be considered self-pay unless otherwise proven. Self-pay  patients will be required to pay in full for services rendered to them and will be asked to make payment arrangements prior to  services being rendered. Emergency services provided to self-pay patients will be billed to the patient.

At the sole discretion of the practice, extended payment arrangements may be made for patients. Please speak with our practice  manager to discuss a mutually agreeable payment plan. It is never our intention to cause hardship to our patients, only to pr ovide  them with the best care possible and reasonable costs.

**I received a bill even though I have secondary insurance.**

Having secondary insurance does not necessarily mean that your services are 100% covered. Secondary insurance policies typically  pay according to a coordination of benefits with the primary insurance.

**What if I have billing or insurance questions?**

**Rudd Vision, PLLC** is supported by a staff of dedicated professionals as well as an outsourced third-party billing service. Our office  staff can assist with most financial questions and help relieve the patient/caregiver of burdensome paperwork. Please ask if you  have any questions about our fees, our policies, or your responsibilities. Occasionally, we will direct you to our third party billing  team if we cannot answer your questions.

**Do you bill workers’ compensation?**

We will bill workers’ compensation for verified claims. It is the patient’s responsibility to provide our office staff with employer  authorization and contact information regarding a workers’ compensation claim. If the claim is denied by the workers’ compensation  insurance carrier, it then becomes the patient’s responsibility.

At your request, we will submit the claim to your primary medical insurance carrier with a copy of the workers’ compensation  insurance denial. If your primary medical insurance carrier’s claim is denied, you will be responsible for payment in full.

**What if I need surgery?**

If your physician recommends surgery, your surgery will be scheduled by your physician’s staff. The staff member can answer  specific questions about the surgery scheduling process, discuss the paperwork and tests involved, and assist with completing all  prior authorization your insurance company might require.

Our office will require a pre-surgical deposit equal to the amount of your copayment/deductible to go toward your surgery  copayment, deductible, or any other amount your insurance carrier deems to be the patient’s responsibility. After your insurance  company has processed your surgery claim, any amount remaining as a credit will be refunded to you.

**I received more than one bill for my surgery/procedure/service** _._

Please note that **Rudd Vision, PLLC** only bills for services rendered by our clinical team during the procedure. The hospital or other  providers may bill you for other services provided—which might include operating room costs, anesthesia costs, other doctor’s  charges, etc. If you believe you have been accidentally billed twice for the same service, please get in touch with our office for  clarification or resolution.

**Do you bill other third parties?**

We do not bill third parties for services rendered to you. Our relationship is with you and not with the third -party liability insurer or  policy carrier (eg, auto or homeowner). It is your responsibility to seek reimbursement from them. However, at your request, we  will submit a claim to your primary health insurance carrier. You will be asked to pay in full for the services we provide you. All  formalities required by your insurer and the third party should be promptly completed by you. If we receive a denial of your claim,  you will be responsible for payment in full.

**What if my insurance pays late?**

As a courtesy to you, we bill your insurance company for services on your behalf. If any insurance company fails to process p ayment  for services within 45 days from the date of the claim submission, the total balance will be determined to be the patient’s responsibility.

**Will I receive statements or bills?**

It is our office policy that all accounts with pending balances be sent statements via the patient portal. You will receive an email  notification of any messaging in the portal, including billing statements. We also have very convenient online bill pay, text-to-pay  options to facilitate your ability to pay your balances owed. Accounts with unpaid balances for 90 calendar days or more will be  sent to an external collection agency or attorney for collection. Unpaid bills can also lead to possible discharge from the practice.

In the event an account is turned over for collections, the person financially responsible for the account will be responsible for the  collections costs, including attorney fees and court costs.

Regardless of any personal arrangements that a patient might have outside of our office if you are 18 years old or older and  receiving treatment, you are ultimately responsible for payment of the service. Our office will not bill any other personal p arty.

**Do you refer unpaid bills to collection agencies?**

If a patient cannot pay the balance on their account according to the financial policy will be referred to an outside collection agency  or an attorney for further action.

**What if my child needs to see a physician?**

A parent or legal guardian must accompany patients who are minors on the patient’s first visit. This accompanying adult is  responsible for payment of the account, according to the policy outlined on the previous pages.

**Do you charge a penalty for returned payments?**

Any charges incurred by the practice collecting balances owed to us during the collection process may be charged to the patient.  Returned checks, credit card chargebacks, or returned payments will attract a minimum $35 penalty in addition to the balance  owed. Accounts with returned payments will be expected to make payments via cash, money order, or cashier’s checks only.

**Can you waive my copay?**

We cannot waive deductibles, coinsurances, or copays that are required by your insurance. This is a violation of insurance rules.

**I have a hardship. How can you help me?**

Some patients may accrue large balances for services provided. At the sole discretion of the practice leadership, we will wor k with  you to set up a mutually feasible payment plan. In some cases, if the minimum payment due cannot be paid, we will need proof of  financial hardship. We may be forced to pursue collections of balances in the absence of tangible proof of hardship.

**Do you charge for completing forms?**

Completing disability forms, FMLA forms, and other requested supplemental insurance forms requires time away from patient car e  and day-to-day business operations. A prepayment of $15.00 per form is required. Please understand that to complete forms, your  medical record must be reviewed, forms completed and signed by the physician, and copied into your medical record. Some of these  forms can be quite complicated and tedious to fill out. Please provide us with pertinent information, especially dates of dis ability

and return to work. We request that you allow 5 business days for this process.

**Do you charge for copies of medical records?**

Patients requesting copies of their medical records will not be charged a fee.

Attorneys and Insurance companies requesting medical records will be charged a $15 fee plus postage and these fees:  • $0.25 per page – under 100 pages

- $0.10 per page – over 100 pages
- $15 for an itemized bill

Expedited requests will be charged a special handling fee.

Records requested via electronic media (flash drives, CDs, DVDs, etc.) will be charged an additional $10 device fee.

**What if I missed my appointment to see the physician?**

We understand that on rare occasions, issues may arise, causing you to miss your appointment when you cannot notify our office  before your appointment. Should you experience any unforeseen circumstance that causes you to miss your appointment, please call  our office at least 24 hours prior to having it rescheduled.

Our highly skilled physicians are committed to your well-being and have reserved time just for you. Patients who miss more than one  appointment without notifying our office 24 hours before the appointment time are subject to a $20 missed appointment fee billed  to the patient.

I have read, understand, and agree to the above Financial Policy. I understand my financial responsibility to make payments for  services provided to me and the courtesy extended by **Rudd Vision, PLLC** to simplify insurance reimbursement for the services  provided to me. I acknowledge that these policies do not obligate **Rudd Vision, PLLC** to extend credit to me for services provided.

**Get a list of those with whom we’ve shared information**

- You can ask for a list (accounting) of the times we’ve shared your health information for six  years prior to the date you ask, who we shared it with, and why.

## What is the Light Adjustable Lens?

The Light Adjustable Lens (LAL) is an innovative intraocular lens implant that stands out as the only FDA-approved IOL capable of being adjusted after implantation. This cutting-edge lens is made from a special material that responds to UV light, allowing for precise, post-surgery modifications to fine-tune vision correction. With the Light Adjustable Lens, up to twice as many of Dr. Rudd’s patients are able to achieve 20/20 vision after cataract surgery, reducing or even eliminating the need for glasses or contact lenses.1

During cataract surgery, the eye’s clouded natural lens is removed and replaced with an artificial intraocular lens (IOL) to restore clear vision. Traditionally, IOLs are selected before surgery and cannot be altered afterward. However, the LAL offers a breakthrough by allowing adjustments after implantation, ensuring a more personalized vision correction experience.

[Refractive lens exchange (RLE)](https://www.lasik-seattle.com/rle-surgery/) is a procedure similar to cataract surgery but performed electively for patients who do not yet have cataracts. How It Works

## The Cataract Surgery and RLE Procedures

Both cataract surgery and refractive lens exchange (RLE) are outpatient procedures designed to restore and enhance your vision, typically taking only 10 to 15 minutes per eye.

- **Preparation**: On the day of your surgery at Rudd Vision, you will spend a 1-2 hours with us to allow for preparation and post-operative recovery. Before the procedure begins, your eyes will be numbed with anesthetic eye drops, and you may receive a mild oral sedative to ensure your comfort. A device will gently keep your eye open, so there’s no need to worry about blinking during the procedure.
- **Procedure**: The surgery starts with the creation of a small incision in the cornea. At Rudd Vision, this is achieved using a femtosecond laser, which offers a higher level of precision compared to traditional methods. The natural lens of the eye is then broken up and removed in a process called phacoemulsification. Finally, the Light Adjustable Lens is inserted. The incisions are so small that they typically self-seal, eliminating the need for sutures.
- **Post-Procedure**: After the surgery, you will rest briefly in our recovery area before being ready to head home. Since you won’t be able to drive, please arrange for a friend or family member to assist you.

## Recovery After Cataract Surgery and RLE

Following your cataract surgery or refractive lens exchange (RLE) at Rudd Vision, you’ll begin a brief recovery period designed to ensure optimal healing. You will be fitted with a protective eye shield, which should be worn while sleeping or lying down during the first few days after your eye surgery. It’s normal to experience some mild discomfort, irritation, or temporary blurry vision initially, but these symptoms typically resolve quickly, allowing you to notice an improvement in your vision shortly after surgery.

Dr. Rudd will prescribe antibiotic and anti-inflammatory eye drops to support your recovery. It’s essential to follow the provided instructions carefully to avoid complications and promote smooth healing. Most patients find the recovery time to be minimal, often feeling ready to return to work and resume normal activities within 24 to 48 hours. However, it’s advised to avoid strenuous activities during the first couple of weeks to ensure proper healing.

Please note that it is imperative that you wear UV-protective glasses every time you go outside following LAL surgery until your lens customization has been completed.

You’ll return to Rudd Vision for a follow-up visit 24 hours after your eye surgery to ensure that your recovery is progressing as expected.

## Light Adjustable Lens Customization

After your eyes have healed from the initial lens implantation surgery, you’ll return to Rudd Vision to begin the customization process for your Light Adjustable Lens (LAL). While these treatments are quick, comfortable, and non-invasive, typically lasting less than two minutes, you will spend about one hour with us at each adjustment visit.During each session, your vision will be assessed, your eyes will be dilated, and a specialized Light Delivery Device (LDD) will emit ultraviolet (UV) light to gently adjust the curvature of your LAL, fine-tuning your vision correction. Because we are treating both eyes, you will typically need a driver with you for each adjustment.

Most patients require between 2 to 4 light treatments, spaced about three to seven days apart, to achieve optimal visual outcomes. This process allows you to “test drive” your vision, ensuring that the final result meets your specific needs. Once the desired prescription is reached, your eye doctor will perform a final light treatment to “lock in” the prescription, providing you with significantly improved vision without the need for further adjustments.

### Am I a good candidate for the Light Adjustable Lens?

The Light Adjustable Lens (LAL) is an excellent option for patients undergoing cataract surgery or refractive lens exchange (RLE) who are seeking highly accurate vision correction. Ideal candidates are those who desire the flexibility to fine-tune their vision after surgery, potentially reducing or eliminating the need for glasses.

You may be a good candidate for the LAL if:

- You are scheduled for cataract surgery or RLE.
- You have not taken medications that cause sensitivity to UV light or retina damage.
- You do not have a history of eye infections, such as herpes eye infections.
- You do not have uncontrollable eye movements.

Patients who have previously undergone vision correction procedures like LASIK or PRK are often particularly well-suited for the LAL, as it allows for precise adjustments that can accommodate the unique characteristics of eyes that have undergone previous surgeries.

### Is LAL customization painful?

No. customization sessions are only a couple of minutes long, and your eye will be numbed before Dr. Rudd begins. Some patients do report a sensation of pressure, but pain is not reported.

### Is it really necessary to wear UV protective glasses after LAL implantation?

Yes. The Light Adjustable Lens is adjusted with UV light, and unintended exposure to UV light before customization is complete may alter results.

### Are There Any Risks Associated with the Light Adjustable Intraocular Lens (LAL)?

As with any surgical procedure, cataract surgery and refractive lens exchange (RLE) carry some risk of complications. However, these procedures are widely performed and generally have a very low rate of complications. For those seeking maximum independence from glasses, this premium option is designed to allow patients to spend 90-95% of their day glasses-free. This is excellent for individuals who want to minimize their reliance on glasses1 for most daily activities and reduce the risk of falls as they age.2 This approach is especially beneficial for those experiencing presbyopia or dysfunctional lens syndrome, as it addresses a wide range of vision needs. Typically, it involves a combination of a multifocal IOL (MFIOL) in one eye and an extended depth of focus (EDOF) IOL in the other. However, Dr. Rudd will customize the combination to best meet your unique needs and lifestyle, ensuring optimal vision at multiple distances.

## Distance or Near Vision Correction

This vision correction option is designed for patients who want their sharpest vision at a specific distance. With this option, Dr. Rudd maximizes your vision through astigmatism correction, along with implanting a monofocal IOL. You can choose to have clear vision for distance, near tasks, or a combination where one eye is set for distance and the other for near (blended vision). Although this option provides excellent clarity at the chosen distance, it requires wearing glasses for other activities such as reading or using a computer. As a result, this option is less common because Dr. Rudd feels it will require bifocal glasses later in life, along with possibly creating some imbalance/depth perception issues that may result in instability as one ages. However, this may be the best option for patients with several underlying eye disorders (macular degeneration, glaucoma, etc.)

## Advanced Vision Correction

Standard, or monofocal IOLs are considered a necessary part of cataract surgery and are covered by insurance. These lenses replace a lens clouded by cataracts and can also improve vision at a single range (near or distance). Because standard IOLs have a single focal point, they cannot correct astigmatism or vision at multiple ranges. Most patients choose to correct their distance vision during cataract surgery, which means they will still need to at least wear bifocals or reading glasses for near tasks. Studies3 have shown that wearing bifocals increases the risk of falls as patients age.4

## What are Cataracts?

Cataracts occur as a normal part of the eye’s aging process. Starting around the age of 40, the proteins in the eye’s lens begin to degrade and clump together, forming cloudy areas known as cataracts. These clumps obstruct light from passing through the lens clearly, leading to increasingly blurry vision.

Cataracts are a common condition, especially as individuals age. According to the National Eye Institute (NEI), approximately 50% of Americans will develop cataracts by the time they reach 75 years old.1 More than 20.5 million Americans over the age of 40 are currently affected by this condition, reports the American Academy of Ophthalmology.2

This clouding of the lens is an inevitable part of the aging process but can be effectively managed and treated to restore clear vision. When cataracts begin to affect a person’s activities of daily living, they must be treated with cataract surgery.

## Symptoms of Cataracts

Cataracts develop gradually, often starting as a minor inconvenience and progressing to more noticeable vision impairment. Initially, the symptoms might be so mild that they are hardly detectable, but as the condition worsens, the following signs become evident:

- **Blurry or Cloudy Vision**: One of the earliest and most common symptoms, where vision appears foggy or blurred.
- **Changes in Color Perception**: Colors may seem faded, dull, or acquire a yellowish tint.
- **Increased Sensitivity to Light**: Lights may appear overly bright, or there may be a halo effect around lights, especially noticeable at night.
- **Night Vision Difficulties**: Driving or seeing in low light can become challenging due to reduced clarity and increased glare.
- **Double Vision**: In severe cases, double vision may occur in the affected eye, making it difficult to perceive images clearly.
- **Frequent Prescription Changes**: A noticeable increase in the frequency of prescription changes for glasses or contact lenses can be a sign of progressing cataracts.3

Cataracts symptoms typically emerge more prominently as individuals age, often becoming more significant in their 50s and beyond. If you notice any of these changes in your vision, discuss them with your eye doctor: a comprehensive eye exam is recommended to determine if cataracts are the cause.

## Dysfunctional Lens Syndrome and Cataracts

Dysfunctional Lens Syndrome (DLS) is a term used to describe the natural degeneration of the eye’s crystalline lens, which affects functional vision as we age. This condition progresses through various stages, ultimately leading to cataracts if left untreated. Understanding DLS can help patients recognize the changes in their vision and seek appropriate treatment.

**DLS Stage I (~Ages 40-50)**: During this stage, the lens begins to lose its ability to focus up close, a condition known as presbyopia. Patients often find they need bifocal segments in their glasses to see clearly at different distances.

**DLS Stage II (~Ages 50-65)**: As DLS progresses, the quality of vision starts to deteriorate. Images may appear less crisp, and colors might seem less vibrant. This stage marks the beginning of more noticeable vision impairment.

**DLS Stage III (~Ages 65+)**: At this stage, DLS progresses to cataracts, where the lens becomes significantly opaque, impairing vision to the point where it cannot be corrected to better than 20/40 with glasses or contacts. This level of impairment is typically when insurance considers the cataract to be visually significant and medically necessary to remove.

Dr. Jay Rudd offers treatments for each stage of Dysfunctional Lens Syndrome. While surgery can be performed at any stage of DLS, it is important to note that in stages I and II, insurance does not typically cover the procedure as it is not deemed medically necessary (referred to as Refractive Lens Exchange, or RLE). However, in stage III, insurance usually covers the removal of the cataract and the implantation of a standard intraocular lens (IOL).

## Treatment for Cataracts

- **Optimal Intraocular Lens Placement**: The laser aids in performing a critical step—creating the capsulorhexis, or the circular cut in the lens capsule, crucial for lens removal and IOL placement. The laser’s accuracy ensures a perfect circle every time, which is essential for the precise placement of the new lens, leading to more predictable visual outcomes with lower4 rates of enhancements or IOL repositioning.

## Preparing for Cataract Surgery

Preparing for cataract surgery at Rudd Vision begins with a comprehensive consultation with Dr. Rudd. This initial appointment is crucial for setting the stage for a successful surgery. During the consultation, an eye exam will be performed to confirm your cataract diagnosis and diagnose any potential eye diseases such as glaucoma and macular degeneration. Once your cataract procedure is scheduled, we will give you comprehensive preoperative instructions to prepare you for the day of the surgery, including any adjustments you might need to make to your daily routine. You will also receive guidance on what to expect during the cataract surgery recovery period.

How It Works

## The Laser Cataract Surgery Procedure

Cataract surgery is an outpatient procedure that takes less than 10 minutes per eye. Before your surgery begins, anesthetic eye drops will numb your eye, and you may also be given an oral sedative to ensure your comfort throughout the process. A special device will be used to keep your eye open, so there’s no need to worry about blinking.

The procedure starts with the femtosecond laser making a very small incision in the cornea. This laser not only ensures exact incisions but also corrects any minor astigmatism. Following the incisions, the cloudy lens of the eye is broken up and removed through a process called phacoemulsification. Then, a [new lens (IOL)](https://www.lasik-seattle.com/iol-options/) is inserted. The precision of the laser and the small size of the incisions mean that they will self-seal, so typically no sutures are needed.

## Cataract Surgery Recovery

After your cataract surgery, you will be fitted with an eye shield to protect your eye as it begins to heal. This shield should be worn while sleeping or lying down during the initial recovery phase. It is normal to experience some initial blurry vision and mild discomfort such as irritation or soreness immediately following the procedure. However, these symptoms should resolve quickly, allowing you to notice a significant improvement in your vision soon after the surgery.

Your eye surgeon will prescribe antibiotic and anti-inflammatory eye drops to aid in your recovery. It’s crucial to closely follow instructions regarding these medications to ensure a smooth healing process.

You can expect to return to Rudd Vision 24 hours after your procedure for a follow-up visit to ensure everything is progressing well. Most patients feel ready to resume their normal activities within 24 to 48 hours of their surgery, though you are advised to avoid strenuous activities for the first couple of weeks to ensure optimal healing.

### Is cataract surgery covered by insurance?

When cataracts have progressed to the point that a person’s vision and ability to perform normal tasks are compromised ( [DLS](https://www.lasik-seattle.com/cataract-surgery/#dls) stage III with 20/40 vision at best), basic cataract surgery is covered by insurance. This includes manual cataract surgery with standard IOLs, which eliminate cataracts and improve vision at a single distance, which means that many patients will still need reading glasses or bifocals. However, if a patient opts for laser cataract surgery and/or lifestyle vision options to further optimize their vision, they will incur out-of-pocket charges.

### What are the risks of cataract surgery?

Cataract surgery is one of the most commonly performed surgical procedures globally and is known for its high safety and success rates. However, as with any surgical procedure, there are potential risks and side effects. While complications are rare, it’s important for patients to be aware of them.

Possible risks of cataract surgery include:

- Infection
- Inflammation
- Swelling or bruising of the eyelid
- Increased intraocular pressure
- Vision changes
- Retinal detachment9

Dr. Rudd will discuss these potential risks with you during your consultation and will take every precaution to minimize them. Understanding these risks will help you make an informed decision about your cataract surgery.

### Can cataracts come back after surgery?

No. During cataract surgery, the natural lens of the eye is removed and replaced with an artificial lens, so cataracts will not return. However, some patients may experience hazy vision after cataract surgery. This is sometimes referred to as “secondary cataracts,” but is actually a condition called posterior capsular opacification (PCO). This can be easily treated with a quick procedure performed in our office, called Yag Laser Capsulotomy.

### When can I drive after cataract surgery?

You may experience a dramatic improvement in your vision immediately after your cataract surgery, but your eye will still be recovering. You should be cleared to drive after your follow-up visit, the day after your procedure.

### Can I have cataract surgery after LASIK?

Yes, undergoing cataract surgery after having [LASIK](https://www.lasik-seattle.com/lasik-eye-surgery/) is entirely possible. However, patients who have had LASIK or other refractive surgeries like PRK may experience more complex planning stages for cataract surgery. The main challenge lies in determining the appropriate power for the intraocular lens (IOL) to be implanted, as previous modifications to the cornea’s shape can affect calculations.

## What is Refractive Lens Exchange?

Refractive lens exchange (RLE), is also referred to as clear lens extraction or clear lens exchange. This vision correction procedure involves replacing the natural lens of the eye with a clear artificial intraocular lens implant (IOL).

The refractive lens exchange surgical procedure is identical to [cataract surgery](https://www.lasik-seattle.com/cataract-surgery/), except that it is performed electively to correct refractive errors rather than cataracts. Refractive lens exchange is an ideal solution for age-related near vision loss, or presbyopia, because it can correct vision at multiple distances—something that other vision correction procedures like LASIK eye surgery or PRK can’t offer.

These refractive errors, which affect how light is bent onto the retina, can lead to blurred vision and other visual disturbances. Here are the common conditions treated by RLE:

- **Myopia (Nearsightedness):** Affecting up to 30% of the population in the United States, myopia makes distant objects appear blurry.1
- **Hyperopia (Farsightedness):** This condition makes it difficult to focus on close objects and can also lead to eye strain, headaches, and burning or itchy sensations in the eyes.2

## The Refractive Lens Exchange Procedure

Refractive lens exchange surgery is an outpatient procedure that takes only about 10-15 minutes per eye.

- **Preparation:** On the day of your surgery, expect to spend a few hours at Rudd Vision to allow for preparation and recovery. Before the procedure, anesthetic eye drops are applied to numb your eyes, ensuring comfort throughout the surgery. You may also receive a mild oral sedative to help you relax.
- **Procedure:** The surgery begins with your eye surgeon making a small incision in the cornea to remove your natural lens. At Rudd Vision, this is done with the assistance of a femtosecond laser for enhanced precision. Then, a customized intraocular lens (IOL) is inserted in its place. The incision is so fine that sutures are not required; the eye heals naturally over time.5
- **Post-Procedure:** After the procedure, you will rest briefly in our recovery area before you are ready to be driven home by a friend or family member.

## Refractive Lens Exchange Recovery & Results

Recovery after refractive lens exchange surgery is brief. When you return home, you’ll need to wear a protective eye shield while sleeping for the first few days to safeguard your eyes. It’s normal to experience some mild discomfort, itchiness, or temporary blurry vision during this initial period. To help prevent any complications, it’s important not to rub or touch your eyes.

Dr. Rudd will prescribe both antibiotic and anti-inflammatory eye drops to use after your refractive surgery. Following their instructions carefully will help ensure a smooth recovery. Most patients are pleasantly surprised by the minimal downtime required; many return to work and resume regular activities within 24 to 48 hours after their surgery.

One of the most rewarding aspects of RLE is the quick improvement in vision. ### Is RLE covered by insurance?

Refractive lens exchange is performed on an elective basis to correct refractive errors. Because RLE is not medically necessary, it is not typically covered by insurance. ### How is refractive lens exchange different from LASIK?

## What is EVO ICL?

EVO ICL™ by STAAR Surgical is an FDA-approved Implantable Collamer™ Lens that is placed between the natural lens of the eye and the iris to improve a patient’s vision. ## Who is a Candidate for EVO ICL?

The best way to determine whether you are a good candidate for EVO surgery is to attend a consultation with Dr. Rudd. In general, ideal candidates for EVO ICL:

- Are ready to reduce or even eliminate their need for corrective eyewear
- Are between the ages of 21 and 45
- Have had a stable vision prescription for at least one year (less than 0.50 difference)
- Have moderate to high myopia (nearsightedness) with or without astigmatism
- Do not have a history of eye disease

Even if you have been told you don’t qualify for [LASIK](https://www.lasik-seattle.com/lasik-eye-surgery/) or other laser vision correction procedures due to thin corneas or other factors, you may still be a candidate for EVO ICL surgery.

Your Surgery

## Preparing for EVO ICL Surgery

Your consultation with Dr. Rudd is key to feeling well-prepared for your EVO ICL procedure. Before this appointment, you will need to stop wearing contact lenses for a period of time, as this can change the shape of your cornea.

- Soft contact lenses should be removed for at least 3-5 days
- Hard contact lenses should be removed for at least 2 weeks

During your EVO ICL consultation, Dr. Rudd will perform a full eye exam and review of your medical history and eye health. He will help you learn more about the benefits of EVO so that you can make an informed decision about your vision correction.

Once you and Dr. Rudd have decided together that EVO is right for you, your vision correction surgery will be scheduled and you will receive detailed instructions for preparing for the procedure. These may include:

- Stopping use of some medications, vitamins, and/or supplements


Arriving at our vision center wearing no makeup, facial lotions, perfumes, or jewelry
- Refraining from eating or drinking for a period of time before your procedure
- Making arrangements for transportation to and from your eye surgery

How it works

## What to Expect During EVO ICL Surgery

EVO ICL surgery is performed as an outpatient procedure at our sterile Rudd Vision office based surgery suite (OBS). The EVO ICL procedure is quick and safe, usually completed in 20-30 minutes. Accounting for pre and post-operative care, you can expect to be at our center for about 2 hours.

Before your EVO ICL surgery begins, anesthetic eye drops will be applied to numb your eyes, and you may also be offered an oral sedative so that you feel relaxed. Your eyes will be dilated, and once you are comfortable, you will be escorted to our Rudd Vision OBS where you will be prepared for the procedure. Once your eye is numb, Dr. Rudd will make a very small incision at the edge of your cornea. Your EVO ICL will be positioned behind the colored part of your eye (the iris) and your eye’s lens.

### How is EVO ICL surgery different from LASIK and PRK?

EVO, [LASIK](https://www.lasik-seattle.com/lasik-eye-surgery/), and [PRK](https://www.lasik-seattle.com/lasik-eye-surgery/#prk) are all safe and effective vision correction procedures, but there are some key differences. LASIK and PRK work by permanently reshaping the cornea to improve the way light is focused onto the retina. EVO ICL, on the other hand, is an additive technology that does not require the removal of any corneal tissue. Because of this, EVO may be a more suitable option for patients who have thin corneas, concerns about dry eye syndrome, or higher vision prescriptions that can’t be addressed through laser vision correction. This also means that EVO ICL can be removed by a doctor if desired.

### Is EVO ICL surgery painful?

Before EVO ICL surgery, your eyes will be numbed with anesthetic eye drops. You should not feel any pain, and the recovery period is typically comfortable.

### Will other people be able to see EVO in my eye?

No. EVO ICL is implanted behind the iris (the colored part of the eye) and is invisible once in position. You also won’t be able to feel your implantable collamer lens.

### Is EVO ICL surgery covered by insurance?

Because EVO ICL is considered to be elective (not medically necessary) refractive surgery, it is not typically covered by our patients’ insurance plans. However, we at Rudd Vision believe that everybody should have access to sharp, clear vision regardless of financial constraints. ### Is EVO ICL permanent?

## What is LASIK Eye Surgery?

LASIK, or laser-assisted in situ keratomileusis, is an FDA-approved laser eye surgery procedure that offers a path to improved vision by reshaping the eye’s cornea.1 This surgical method addresses common vision problems—refractive errors—caused by discrepancies in the cornea’s curvature:

- **Myopia** (nearsightedness) occurs when the cornea is overly curved for your eye.
- **Hyperopia** (farsightedness) happens when the cornea is not curved enough for your eye.
- **Astigmatism** arises from an uneven cornea, shaped more like a football than the spherical shape of a basketball.

These irregularities in the cornea’s shape interfere with the way light rays are focused onto the retina, leading to blurred or distorted images. By finely adjusting the shape of the cornea, LASIK surgery enables a correction of these imperfections. Is LASIK for You?

## Who is a Candidate for LASIK?

If you are interested in greatly reducing your dependence on glasses or contact lenses (or getting rid of them altogether), LASIK may be the right laser vision correction procedure for you. During your consultation, Dr. Rudd will perform a thorough evaluation to determine whether you are a good candidate for LASIK.

In general, a good candidate for LASIK:

- Is over the age of 18
- Has a stable vision prescription, with minimal changes in the last year
- Has corneas of adequate thickness
- Does not have eye conditions like glaucoma or keratoconus
- Does not have a history of eye infections
- Is in good overall health4

Some of our patients are concerned that they can’t have LASIK because they have astigmatism, or that they are too young or too old for LASIK. This is not always the case: the best way to determine whether you are a good candidate for LASIK is to schedule a consultation with Dr. Rudd.

Your Surgery

## Preparing for LASIK Eye Surgery

The most important part of preparing for LASIK is your consultation with Dr. Rudd. During this appointment, he will perform a thorough eye exam. He will also review your medical history, eye health, and other factors that will help determine whether you are a good candidate for LASIK.

Once your LASIK surgery is scheduled, a member of our team will give you detailed instructions to follow in the days leading up to your procedure. These will include:

- Stopping use of certain medications
- Stopping contact lens wear
- Arriving at our center with a clean face, free of lotions, creams, or makeup

You won’t be able to drive yourself home on the day of your LASIK surgery, so please arrange for transportation with a friend or family member.

How it works

## What Happens During LASIK Surgery?

It might surprise you to learn that the LASIK procedure that will transform your vision takes only a few minutes.

Before LASIK, anesthetic eye drops will be used to numb your eyes. You may also be offered an oral sedative to help you relax. In the first step of LASIK surgery, Dr. Rudd creates a very small flap in the top layer of the cornea. Next, he reshapes the corneal tissue, using an advanced excimer laser that allows for extreme precision. Once the cornea has been reshaped to correct refractive errors, the corneal flap is put back into place. The corneal flap is so small that it will seal itself—no sutures are required.

Get back to your Every Day Life

## Recovery After LASIK Surgery

When you return home after your LASIK procedure, you will likely rest for the remainder of the day. You might experience light sensitivity and/or minor itching or burning: this is normal and will resolve itself within a day. Dr. Rudd will send you home with eye shields to wear when you sleep, as well as prescription eye drops. You may also experience dry eyes, and this can linger for some patients. Dr. Rudd will recommend lubricating eye drops.

Very little downtime is needed after LASIK—most of our patients feel comfortable going back to work and normal activities the very first day after their LASIK procedure!

You will see Dr. Rudd for a follow-up the day after your LASIK procedure, and he will advise you to wait one week before exercising or engaging in any strenuous activity. You will need to refrain from swimming for a week after LASIK surgery.

## FAQs About LASIK

### Does LASIK hurt?

No. Your eyes will be numbed with anesthetic eye drops before your procedure, so you should not feel any pain. You may feel slight pressure during the creation of the corneal flap and you may feel a minor discomfort, irritation, or burning during the recovery period, but pain should not be a part of your experience.

### Is LASIK safe?

Yes. LASIK is FDA-approved and has been certified to be both safe and effective. According to the American Refractive Surgery Council, fewer than 1% of LASIK procedures have complications.6

While LASIK has an exceptionally low complication rate, there are risks and side effects associated with every surgical procedure. According to the American Academy of Ophthalmology, the most common side effects of LASIK are:

- Dry eyes
- Halos glare, and starbursts at night
- Double vision
- Over- or under-correction of refractive errors
- Dislocation of the corneal flap
- Light sensitivity7

### Is LASIK permanent?

LASIK permanently reshapes the cornea to correct refractive errors like nearsightedness (myopia), farsightedness (hyperopia), and astigmatism. ## Recovery After SMILE pro

After your SMILE pro surgery, you’ll need someone to drive you home. For the first 24 hours, avoid activities that strain your eyes, such as reading, watching television, or using the computer. One of the advantages of SMILE pro is the quick recovery time, allowing many patients to return to work and normal activities the next day. You will be scheduled for a follow-up visit with Dr. Rudd to monitor your healing the day after your vision correction surgery.

## Jay Rudd, MD

Dr. Jay Rudd has been in private practice as a fellowship-trained cornea, cataract and refractive surgeon since 2000 and has performed thousands of vision correction procedures for the residents of Olympia, Lacey and beyond.

## Education

Dr. Rudd received his Bachelors of Science at the University of California, Los Angeles followed by a Doctor of Medicine at Jefferson Medical College (now Sidney Kimmel Medical College) in Philadelphia. He completed his residency at the prestigious Wills Eye Hospital, consistently ranked among the top ophthalmology programs in the country. Dr. Rudd further specialized in Cornea and Refractive Surgery during his fellowship at the John A. Moran Eye Center at the University of Utah.

## Professional Memberships

Dr. Rudd is a founding fellow of the World College of Refractive Surgery and Visual Sciences and an active member of several professional organizations, including:

- Refractive Surgery Alliance
- American Society of Cataract and Refractive Surgeons
- American Academy of Ophthalmology
- Washington Academy of Eye Physicians and Surgeons
- World College of Refractive Surgery and Visual Sciences

In 2018, Dr. Rudd completed the Physician CEO program, gaining advanced business acumen to complement his medical expertise.

## Research and Academic   Contributions

Dr. Rudd has contributed significantly to the field of ophthalmology through his involvement in numerous FDA studies over the past two decades, primarily focusing on multifocal intraocular lenses for Cataract Surgery and Refractive Lens Exchange (RLE). He authored the “Refractive Surgery Complications” chapter in the renowned textbook, _Duane’s Clinical Ophthalmology_, published in 2000.
